Description

The app server docs should be updated to match the plugin. They are a mess of J2EE plugin references

This fixes all (I think) of the documentation. Let me know if I missed anything. It also adds support for Jetty-4.x, when combined with the other attachment.

the patch adds jetty, but there is a separatent jetty plugin for this. I'm not convinced the documentation improvements are that great - some still seemed incorrect and it looked mostly reformatting.
