Funny, I used various mapserver generated indexes lately and they all worked fine?

Did you try the attached ones? I remember this issue it was a special case that caused the map server to create a strange index. Look the .shp is only 887Kb but the index is 5.48 MB. More than 6 times the size of the shapefile... But it was generated by the mapserver shptree.
