Description

The vacation request form as found in the user guide (http://activiti.org/userguide/index.html#forms) is not correct. It's missing all the required html elements to render it correctly.

It is missing a lot of formatting so it is rendered like the attached image.

The right example can be found in the vacationRequest example. See attached file request.fomr
